About the role

Do you want to use your skills and expertise to really make a difference and help support people with mental health problems? If so, this could be a great role for you.

We are seeking an enthusiastic Dementia pathway Facilitator to join our Remind dementia service. As a key player in our dementia service, you will providing essential support to individuals with dementia and their carers.

Any offer for this role is subject to a Disclosure & Barring Service (DBS) Check.

About you

Collaborate with a diverse group of professionals, including our supportive Team Leader, dedicated Intensive Engagement Support Workers, local stakeholders, and community partners. Your empathy, resourcefulness, and knowledge of dementia support pathways will be essential in connecting clients and their families with the right resources to enhance their care and well-being.
